COMEDK PGET 2015 Application Form begins on November 10, 2014

COMEDK PGET 2015 Application Form - The request for COMEDK PGET 2015 application form begins on November 10, 2014. Candidates after registering for COMEDK PGET 2015 Request for Application form can apply from the same date.

The last date for applying COMEDK PGET 2015 application form through online mode is December 22, 2014. Application in physical format must be received in the COMEDK office on or before December 27, 2014 (before 5.00 PM). The Application form in the physical format should be sent only by Speed Post and any other mode of delivery is not acceptable.

According to COMEDK PGET 2015 eligibility criteria, only those candidates shall be eligible for COMEDK PGET 2015 who has passed their MBBS/BDS from Medical Council of India (MCI) / Dental Council of India (DCI) recognized institution and completed or completing their internship by April 30, 2015.

  

Instructions for filling up of COMEDK PGET 2015 Request for Application Form (RFA): 

·         Name of the Applicant and Date of Birth as it appears in your SSLC/10 Marks Card

·         Select the course either Medical / Dental

·         Unique ID Proof - Any of these will be accepted as ID Proof for filling up application form (Voter ID, Passport, PAN Card, Driving License or Aadhar Card). The same ID Proof must be produced in original during Entrance Test and Counseling

·         Enter your valid Email ID and Mobile number for all correspondence. An Email ID once used for Registration for sending the Request for Application (RFA) cannot be used again

·         The Candidate must click on the declaration before submitting the COMEDK PGET 2015 Request for Application Form. Candidates must ensure that they press ‘CONFIRM’ button only after they are sure that all the details given are correct

COMEDK PGET 2015 Application Form:

COMEDK PGET 2015 Applicant login will be activated only after the successful submission of Request for Application (RFA). Candidates can login to the system by entering their USER ID and PASSWORD as received in their E-mail ID after the successful submission of RFA. After the login is successful, the candidates get an opportunity to not only fill up Online Application Form (OAF) but also, can check the status of Application at various stages.

          

Instructions for filling the Online Application Form (OAF)

·         Candidates are advised to go through the instructions carefully before filling up the online Application Form

·         The candidates should note that the details filled up by them in RFA will be available as a static content and the same cannot be edited

·         Candidates are advised to fill their personal detail as per the column given in COMEDK PGET 2015 application form

·         Applicants have to enter COMEDK PGET 2015 examination fee detail

·         Candidates must fill their academic details such as Name of the Institution Last Studied, MCI / DCI Registration Number, Internship Completion Details etc. in respective column

·         COMEDK PGET 2015 applicants must enter their total marks obtained in each year during the course of study in MBBS / BDS

·         Candidates must sign the declaration form and put their photograph and signature at the respective place in the COMEDK PGET 2015 application for

COMEDK PGET 2015 Application Fee – The cost of COMEDK PGET 2015 application fee is Rs. 2,500. The fee has to be paid in the form of a crossed demand draft drawn on a Nationalized Bank and payable to ‘COMEDK’ at Bangalore.

Dimension for Photograph and Signature for COMEDK PGET 2015 application Form:

Dimension for Photograph – Photograph should be of180 x 220 Pixels and maximum file size should be 200 kb. The image should be of .jpg or .jpeg format

Dimension for Signature – Signature should be of 230 x 90 Pixels and Maximum file size should be 200 kb. The image should be of .jpg or .jpeg format

List of Documents to be sent along with COMEDK PGET 2015 Application Form. Application form without any of the below mandatory documents will be rejected.

·         Demand Draft of Rs. 2500 from any nationalized bank in favour of ‘COMEDK’ payable at Bangalore. Candidates must write their application number & applicant’s name behind the DD

·         Printed Online COMEDK PGET 2015 Application Form

·         Photocopy of SSLC / 10 STD Marks card

·         Photocopy of the Unique ID Proof mentioned in the COMEDK PGET 2015 Request for Application

·         Photocopies of All mark cards / Consolidated Marks Sheet including that of failed year if any (First Year to Final Year university examination marks card/s)

·         Photocopy of Attempt Certificate is a statement indicating the number of attempt one has taken to clear a particular subject/year

·         Photocopy of Internship Completion Certificate / Internship Completed Certificate

·         Photocopy of Degree Certificate / Provisional Degree Certificate / Passing Certificate / Course Completion Certificate

·         Photocopy of Registration Certificate / Provisional Registration Certificate or a receipt / an acknowledgement issued from the competent authority against applying for Registration Certificate

·         Candidates belonging to SC / ST (All India) and OBC of Karnataka would be considered, provided they furnish the duly attested Photocopy of the caste certificates issued by competent authority

·         Candidates who have not produced appropriate supporting documents for claiming the status of Religious / Linguistic Minority / SC / ST / OBC will be rejected

The completed Application Form has to reach COMEDK office by December 27, 2014 (before 5.00 PM) through speed post only:

Important Note:

·         Candidates should take printout of the COMEDK PGET 2015 Online Application Form in A4 size paper and in portrait format.  It is advised to note down six digit application number for future correspondence

·         The envelope containing COMEDK PGET 2015 application form along with enclosures shall also be in A4 size only and the same shall not be folded

·         Candidates must avoid stapling of Demand Draft to the application form, instead use clips

·         Completed application forms will not be received in person or through private courier either in COMEDK office or any member institution
